Our Preventative Wellness Membership Program helps eligible Medicare patients manage chronic health conditions with personalized support between doctor visits. This program is part of our commitment to keeping you healthy, informed, and connected to care – all year long.

We partner with ChartSpan, a trusted care coordination service, to provide monthly check-ins and 24/7 access to healthcare professionals. You'll know it's ChartSpan calling, but the program is part of your care team right here at Rice Medical Associates.

What You Can Expect:

• One-on-one monthly phone check-ins with a ChartSpan care coordinator
• 24/7 access to a team of healthcare professionals – available 365 days a year
• Help managing medications, appointments, and referrals
• A personalized care plan based on your health conditions and goals
• Direct communication with your primary care provider when needed

You’ll never be overwhelmed with calls – you’ll only receive one call per month, and you're welcome to call in anytime you need help or have questions.

Is It Required?

No. Participation is completely optional, but we strongly recommend it for Medicare patients with two or more chronic conditions. It’s a simple, proactive way to manage your health and prevent complications.

You can also unenroll at any time with no penalty.
